Types of Powers of Attorney
There are two types of powers of attorney:
•personal care*
•property
*Personal care. An attorney for personal care can make decisions about your:
•health care
•housing
•other aspects of your personal life such as meals and clothing

Sworn Statement for a Family Gift of a Used Motor Vehicle in the Province of Ontario:
In Ontario you may be allowed to transfer ownership of your vehicle to a family member without paying the retail sales tax. This can be done at a Service Ontario Centre.
What documents are required? You will need to bring the following documents to a Service Ontario Centre:
•A completed Sworn Statement for a Family Gift of a Used Motor Vehicle in the Province of Ontario form
•Proof of Ontario vehicle insurance
•Your Ontario driver’s license
•Safety Standards Certificate (this is not required for spouse-to-spouse transfers, but you must complete a spousal declaration)
•Vehicle Ownership Permit with the completed portion on the back
•A completed plate transfer declaration if you are also transferring the license plate (only permitted between spouses, parents and children)
Only one exempt transfer of the same vehicle, between family members, is allowed within a 12-month period. Find more information on the link below:

Recommended consent letter for children travelling abroad
The signing of a consent letter may be witnessed by anyone who has attained the age of majority (18 or 19, depending on the province or territory of residence). However, we strongly recommend that you have the letter witnessed by a notary public, so that border officials will be less likely to question its authenticity. Invitation letter

Custodian declaration form
The two pages of the form look similar, but: the first page needs to be signed by the custodian and notarized (certified by a lawyer or notary) in Canada and the second page needs to be signed by the parents or legal guardians of the minor child and notarized in their home country

The Statutory Declaration of Progress Payment Distribution by Contractor is a sworn statement for use by the Contractor as a condition of receiving payment for either the second and subsequent applications for progress payment or the release of holdback funds. Rin Letter
A notarized letter to register a vehicle in the name of a corporation. Yon needs to write a letter to the Ministry of Transportation saying you are an authorized person on behalf of the company. There are some requirements to mention on the letter and we recommend you call the Ministry of Transportation and ask for the template of the letter.
